This registration form is intended for every child who attends the center. If you are registering your child at Cooks Hill Community Childcare Center, this is your first form.

These forms are only needed for a new registration starting in the Infant classroom.

Parent Handbook
This is our Parent Handbook. Please review our policies and procedures for Cooks Hill Community Childcare Center. Every family needs this packet, as there is a required signature needed on the last page that gets turned in with registration paperwork.

Cooks Hill Community Childcare Center (CHCCC) is a non-profit Childcare Center, owned and operated by the Cooks Hill Community Church as a ministry to the community, and is licensed by the State of Washington.
